In accord with the Diocese of Rockford's updated Safe Environment Program, all adult parish volunteers (18 years and older) are required to read and sign the following documents:

• Authorization to Conduct Background Check (fill out and return)
• State Of Illinois Mandated Reporter (fill out and return)
• Sexual Misconduct Norms (sign and return last page)
• Code Of Pastoral Conduct (sign and return page 16)
• Code For the Pastoral Use of Technology and Social Media (sign and return page 5)
• Guidelines For Youth and Those Working With Youth (sign and return page 10)
• Additionally, all adult parish volunteers are required to take Protecting God's Children
  Training which can be completed online by going to the following website:
